On the receiver there is a hard button to select "monitor out" (main, sub, both, etc)

I have looked through the IR Database and can not find such a code in the 5 menus in the database? Can someone help?

Or is this something that would have to be set up as a macro going through menu steps

Is worth mentioning as well, if you have the capability (MSC-400) I'd recommend going with serial control instead as the serial protocol supports far more in the way of discrete codes.
