I've used the type for flush-mounted electrical receptacles -- I can't remember the exact price, but they were somewhere around $50 each, at Graybar. For the RJ-11/RJ=45's, Leviton makes a plate that's the same shape as two obround AC outlets, but accepts quick-port inserts. It worked out very nice...
