The good thing is there are lots of pours scheduled and I have the time and budget to create mock-ups that can be test poured to see what happens.
Could you make a small plywood box that’s the same size as the box would be that you will insert after? Lightly glued together. Small enough in the outside dimensions that the plate will cover it.
Glue it to the form. Make a hole in the back or bottom of it for the pipe to enter with a friction fit.
After the form comes off it can be broken out of the concrete. Leaving the appropriate sized recess for you to plate over ?
