How long of a Cat5 can I run to a TV to get RS-232 control? I know the spec is 50', but what have you guys gotten to work out in the real world?
Or should I be running something other than Cat5 for this?

What distance are you looking to do here?? Can you get a processor near this display?
Remotes can talk to several processors at once...
Over 50' feet and I am not feeling comfortable...

Well, I was just looking at this as an alternative to sticking an emitter on the front of the TV. The next job I'm doing will just be 2 plasmas in a basement remodel, so I may be OK with the wire length. I see people talking about 'cracking open' the TV to put the emitter inside... but that scares me a little.

Taking the bezel off most plasmas to put an IR emitter inside isn't a big deal, done it lots of times. I've gone 50' with RS232 on cat5. If you have a problem, there are some products that can extend the RS232 signal. The BTX catalog has several products for just that.

If you have a communication problem at over fifty feet you should be able to reduce the baud rate for the display and the RTI to correct the problem.
Slower rates can go longer distances.

Recently did a job with 5 T-1s and a XP-8, for one zone there is about a 35m (110ft) run between the XP-8 and the Denon AVR-3808 and RS232 works fine via RS232 port. The rest of the equipment is run via I.R. (High Out), there is also a RM433 located with this equipment. No Problems.

Geffen makes a RS-232 to cat-5 adapter that is good up to 300m, and it is less than $100. It is small and very easy to use.

i have used cat5 at distances well over 100' with the cm232s. if you are concerned try running a shielded 22/4 which will improve range and reduce noise. If it is just a TV then there wont be many packets being sent and you shouldn't have any problems with it. I would say if your run is less then 150' but I would run a spare 22/4 shielded just in case if you are worried about it.
