If you select the tv input by cycling through the list (as opposed to one command that goes directly to that input) then you must make sure the list of inputs in the harmony exactly matches the list of inputs in the tv menu - same number in the same order (labels don't matter). Otherwise the harmony thinks it's on one input when it's on another.
