I also think you should delete the TV device since you have been mucking around too much with its configuration. When you delete a device, the affected activities will show a "Set Up Now" or "Need Setup" option. Just click it to fix the activity. It's like running the activity wizard again.

You should also reduce the power on delay of the TV device. The default is 10 sec. This means the Harmony won't send any command to the TV until that time as elapsed from a cold start or after pressing the Help button.

You can determine the required power on delay by using the original remote and a stopwatch. Starting with the TV off, turn On the TV, wait say 3 sec, then send a next input command. If the TV did not change input, then you need more than 3 sec. Else, less than 3 sec. Repeat this process in increment/decrement of 1 sec.
