look at Audio Authority's 985U. It is a DA with one in each of: coax audio left and right analog audio composite video component video I don't remember if it has S, too
with four outputs of each and a cascading output so you can hook up more 985Us. You can use the cascading output as a fifth output if it will ALWAYS be terminated.
