And, of course, you also tried it grounded only at one end, and then grounded only at the other end.

Just for the hell of it, does your TDR show the wire to be the same length for each pair, and from both ends? A bad kink could show different lengths. If it were bad enough to show up on a TDR, it could mess with the data.

Check all the conductors for short to power ground, both when plugged into the network and when not.

Do networks isolate all wire conductors from power ground? I think so but I'm not sure.

In other words, test things you have zero reasons to test.
