Is the ISP-provided box a modem or a router? If a router, I'd check that DMZ is enabled and properly aimed at the Airport device next.

The Spectrum box is just a modem- if I wanted to continue to deal with the Apple stuff, I would get a different router and use the Airport Extreme as a WAP, but I want to get them into something better and this is as good a reason as any.
