I'll assume that you get a RS 15-133 or 134 or 135. Internally these have identical firmware, but the 135 has the most buttons, and the 133 the least.

Assign setup code 1410 to the TV button. I'll use the Menu button as an example.
To program an individual button: (Make sure the remote is in TV mode.)

1) Press P until the remote blinks twice
2) Enter 994 (2 blinks)
3) Tap P
4) Enter 00005 from the below listing
5) Tap Menu
The remote should blink twice to indicate success.
