I am searching for a remote TV control that can be used by a person with very limited hand/finger mobility. I have tried the 'voice activated' type as well as the 'large button' ones. The 'voice' one was hard to program and didn't stay activated without hitting the 'active' button which was difficult for this person to do. The large button one was better, but the buttons had to be pushed with greater force than this person has.

have you thought of touch screen? The Pronto is not cheap, but you can make buttons of any size, and all you need is a touch (not a press). Though it will take some time to get the interface to look like what you want.
