I am thinking of purchasing a Pronto, but have one key question. If I program a macro to turn on five components at the same time, how long a pause is there between each IR code sent out? In other words, do I have to hold the remote up for 5 seconds while it sends five commands or does it happen very quickly?

I've coded a button/macro to turn on a Preamp, TV, DSP Unit and VCR, and it all happens in an extremely short amount of time. The pronto allows you to code delays in macros if you wish (in increments of 1/10 second). It seems to me that the IR codes are sent out in a stream - no pauses, unless you code pauses in.

There's almost no pause. The Pronto is by far the FASTEST macro remote out there. Indeed, it is even possible to get user-configured delays as short as one one-hundreth of a second by using beeps with no volume.
