I want to know the chances to adapted a joystick in the remote, because the user will be an old man that can't move his arms.

With some skill, you could probably adapt a joystick to work with a Pronto 3000 or a Marantz RC9200. Both of them have cursor pads. In theory, you could jack straight into the contacts for those buttons and wire up a joystick.

Or you could try something like the original playstation, where you could get a stick-on joystick which fitted onto the cursor pad.

Now there's an idea. Locate one of those "screw-in" joysticks that some third-party joystick companies use to convert their joypads to joysticks. Then drill a hole in the "joypad" of the TSU3000 or RC9200 and screw the joystick in there. Add a dab of glue to hold it in place.
