I'm using the xbox 360 Harmony remote and was wondering if there was any way to set different starting volume levels. For example I usually need my TV volume set to about 55-60 when watching TV or DVD's but for some reason when playing the 360, I need it set to 28.

Not unless your TV supports discrete commands for setting a specific volume level. The problem is there's no way to tell reliably where it was before.
Check the Xbox and see if it has an output level setting - you may be able to adjust it to match the TV and DVD.

Could he make an activity that would consist of about 22 clicks of the down volume button?

Yes, but that would take a long time to execute and it might not be reliable due to key repeats, etc. Plus you don't want it to execute the volume increase all the time. e.g. When going from Xbox to TV you need to increase the volume but not when going from DVD to TV. Decreasing the volume when going to Xbox would probably work.
