Hi, I am trying to get a file onto a WPK4 Waterpad from Dragon Drop 2.3[Windows 10 PC] without success, can it be done with just the 3 pin com port adapter or do you have to have the programming module?

I did a bunch Xantech keypads back in the analog age. You don't need the PCIR Programming Module to load your file to a keypad.
It is just a crude IR learner and in Xantech's world view it was convenient to leave it connected to the pc serial port and loop from it to the keypad. It's nothing more than a loop-in box in that respect. Kind of handy as you inevitably would need it again to relearn (m)any IR commands that failed to work when the keypad was tested.

Your issue is much more likely to be with the COM port assignment in Dragon, some other Windows 10 driver issue or with your choice of USB / RS-232 dongle. There was a whole lot of discussion on that issue on this very forum like 15+ years ago. More than a few times I would get comms working by just unplugging and reconnecting the 3 pin serial port on rear of the keypad a couple times.

Watch your cost on this as it's really easy with a project like this to come near the cost of replacing the hardware with something from this century. This is especially true if the old stuff can't do exactly what's needed; that would provide an incentive to change to the client to dump the Xantech.
