This past week i had to do a glorified move and Hang and bang for 6 TV's. All was going really well, we discussed exact locations and placement and was given the house keys along with the other contractors todo what we all needed todo.
On the last day (2.5 day project) the client said that the living room TV was all wrong and needed to be redone even after explicitly discussing the height and side to side placement on the first day. It wasnt that big of a deal to move the TV to the other side of the wall and the client ended up fessing up that it was his mistake and had no problem paying for the extra hour it took to move everything over.
Although it hasnt been an issue in the past i would like to get a form that the customer agrees to the location of the TV or speakers and then signs off on the final install to protect all parties.
Does anyone have a form like this made up that they would be willing to share?

Maybe attach a piece of graph paper to the contract? Sketch the room, mark the location and have the client initial it.
A gate contractor i met with last year had that. He sketched out the proposed fence and had me initial it.
We usually have the client or ID mark exactly where they want things with blue tape. For TVs etc we make cardboard cut outs and pin them to the wall so they can visualize the size/height etc.

I would say take a picture of the location on your phone and mark it with Skitch or some other program. Depending on the program you can have them initial or sign. You can have something in a small contract that refers back to the image.

Graph Paper is perfectly fine. My deck guy just did this. I had the same issue with a current client. I moved it once for free the inevitable second move (back to original location) costs them money.
The wife made us move everything at least once.
I will be doing the graph paper next go round. Blue painters tape is always good.
