I have a particularly interesting and difficult problem with MSC-400. This is the first time I am integrating a CISCO U-Verse DVR box. I am using MSC-400 with MX-880, a winning combination I have used on other projects but never with U-Verse.
My problem: I can send IR codes to U-Verse without a problem from MX-880 through MSC-400 to MSC-400 connected emitter. Works every time, reliably and with minimal delay. However, sending U-Verse codes from MSC-400 macros (say System Power On) that are triggered from MX-880 does never work. This forces me to add all U-Verse code at the MX-880 level in addition to triggering MSC-400 macros. Makes for a very messy and confusing macro structure.
I have checked and double checked all I could think of: - physical cable connections including switching ports - using different emitters and changing emitter power - port to emitter associations - deleting U-Verse device and re-adding - re-savings codes from database.
Still does not work. Any ideas would be much appreciated as I am running out of options here. I have several other IR devices that work directly from both MSC-400 and MX-880.

I have UVERSE and a MSC-400 and use macros from the remote and the ms400 and dont have this issue. it could very well be your cable box is the issue. If it were me, id just do it the way it works for the time being.

I have exactly the same problem with my U-Verse Cisco box. Like you, I have tried numerous things with no success. It is odd that the IR code will pass through the MSC-400 then out to the Cisco box, yet when triggered via a Smart Macro, nothing happens. I have check the emitter on the Cisco and it does not flash, the code just seems to dissappear. I wonder if 'mistachy' has a Motorola box, that might explain why his works.

After much frustration I finally found a way around the MSC-400, U-verse issue. I learned every U-verse command directly into the MSC-400. Then set up Smart macros for every command, in addition to my actual Smart macros for power , etc.. On my MX-980 I assigned a Smart macro trigger to each key. Now every button press sends a trigger to ther MSC-400, which in turn sends the learned command to the U-verse box. Works extremely reliably. I also used the learned commands for Favorites. Learning the commands into the MSC-400 went fairly quickly once I got the hang of it. BTW, just to be sure, I used the double learn feature for each command.
