The buttons need to be on top of your background picture of the remote, otherwise there is no way for you to click on them in the remote. I've done just as your suggesting with regards to the cable box remote for my TSU7500 PCF as seen below.
The DVR page above originated from a scan I took of the actual remote. From that scan I created a background image in Photoshop of the DVR section of the remote, then selecting each button area of the background image and cropping to create each of the buttons for later import into PPENG. While the process I followed was a bit more involved than that those are the basics. In the end my DVR page turned out identical to the original remote, but sized to fit the available screen area I had to work with. Also, when pressing a given button on this page said button becomes a little bit smaller as to give you a visual indication that the button is being pressed in.

here. In this case I have the buttons light up red when pressed on the remote. Turned out really well.
