I recently purchased a Harmony Ultimate and I was wondering is there any way to create different rooms above the activity level? The first screen would list the room options ( family room, kitchen, basement). After selecting the room the screen would display the activity options for the room.

No, Harmony remotes are only designed to work at One Activity at a time, not possible to do what you have listed.
