I have had this problem for years on each of my Cablevision SA 8300HD dvrs. I decided to try and determine if there is a setting to change the behavior. When I hit the ff button during playback, the dvr randomly chooses the ff speed. Is there a way to force it to select one of the speeds all the time or, at a minimum, choose the slowest speed all the time? It is really frustrating when it chooses the fastest speed and bolts well past the end of commercials.

I used to have the same/similar problem. The "Replay" button would not reset the FF speed, but the "Play" button did. So my solution was to precede the "Replay" command with the "Play" command. At the time, the activity sequence feature did not exist, so I taught the Play + Replay commands as a single command in RAW mode, and mapped it to the Replay button.
Note: In my case, the bug was fixed (a while back) by one of the many SA8300HD firmware auto updates.
