Admittedly, didn't search very long, but is there a unit out there with this feature.

Customer is pairing a very nice but old Meridian system (like 15 years old) with an Epson projector.

I'm using the optical inputs off of an Xfinity box and a Roku, but the lip sync is of course off.

Any suggestions very much appreciated!

Don't know about adjustable delay, but otherwise might be perfect for you.

Nice.
I called Vanco and they mentioned the new model EVSW1042.

They said they test it specifically to see if the audio will stay in sync. I'll take that with a grain of salt since it's HDMI, but this is a great piece. They claim it supports Dolby Digital and it does 4K

I didn't notice any lip sync issues when I used it.  I had it switching between AppleTV, DirecTV, and a Bluray player.  The analog audio output fed a URC DMS100 amplifier connected to a 2.1 speaker setup in the smaller staterooms on a yacht.

Yup. I use something similar for every Roku installed that goes thru audio distribution.

Well, in this particular scenario the switch did not work so well.

System: 4K Roku, old ass Meridian system, Epson projector, ethereal HDMI HDBaseT extender. Optical out from Vanco switch to Meridian.

First off, no video would even come through without adding an HDMI splitter between the switch and extender.
Second, after burning a crap load of time, audio was still not synced.
