I’ve got a client who blew a fuse on an old, 80s era audio power-amplifer, an SAE A201; the fuse is an “MDX 5” (5 amp, 125 V), yet he’s not been able to find a source, online or local, that sells the fuse.

Any leads on where one could obtain this fuse?

Thank you!

MDA-5 is a direct replacement. 5a is 5a whether its 125v or 250

The voltage is not important. The current rating and the physical size of the fuse of the governing factors. Sometimes the auto parts store has fuses, sometimes the RadioShack will have something like that.
There is no truth anymore. Only assertions. The internet world has no interest in truth, only vindication for preconceived assumptions.

Or an auto parts store.

Buy a few and disconnect the speaker wires from the amp before turning it on- if the fuse blows then, it could point to the power supply or the output section.

These fuses are designed for heavier inrush current that would be typical for small motors or the main fuse for a big amplifier. A truly fast blow fuse probably would not last very long in such applications because repeated starts will fatigue the fuse, but you might be able to use a fast blow for a few tests.

If the glass is seriously blackened, amplifier failure is likely and you should check for output transistor shorts and similar before applying power. This will minimize overall damage. After a while one can begin to "read" fuses. If I see a unit with a seriously blown fuse, I won't apply  power until I have repaired the unit. The first time that I power up, the unit will be operating correctly.

If the glass simply has a small, pale silver stripe, the unit is likely OK and the fuse is being a pain in the neck or incorrectly specified by the designer. I've seen units that would blow the main fuse every few months and there was nothing wrong -- someone specified the wrong fuse. It could have been an inexperienced designer or a purchasing agent who got a deal on a cheaper fuse. All 5A fuses are the same, right? (Not!)

Is there an echo in here?
