Who has dealt with these on AT$T/Pace/Google TV boxes...?

What exactly are they and how best to deal with them, programmatically?

Install an IR system and have the client use the OEM remote.
The local FiOptics company here is doing the same thing.

AT&T boxes suck period. There is no consistency between the pos units
The best answer is to tell them it sucks and to use a good product

Yes. I'm in a Verizon area and we specify FIOS, "don't bring the cable boxes". TIVO is purchased and there is a small monthly site fee for the guide services. Overall, it nets out less than renting multiple cable boxes.
