We have been using the Learn a new remote feature and used a Marantz(Just the one we choose) DVD code set and learn them in to the ATV.
That remote feature gives you Chapter +/- and Advance/Replay as well.
Just use some codes that you want and learn them in. Done. Problem solved.

Sure I guess. If the bare minimum is fine and you don't care if the client has a feature that is available. But I guess that did answer the question.

Yes. Must remember that the advanced functions don't have default hex code assignments (at least none that anyone knows about).
