I've been using Activated Designs for most lifts for the last couple years and also some custom stuff with good results.  Did a 46" tilt down in a master bedroom (what you're asking about) last year and it turned out well.

I've done a few Nexus 21 lifts in the past and liked them, but at the time they only really had pop-up & pop-down lifts which I think might still be true?

I tried Activated Design when I needed a tilt down mechanism and have stuck with them and have now installed their pop-up, tilt-down and even an underbed lift that had an extra extension engineered to clear furniture at the foot of the bed.

Activated Design also has options for projector lift and exterior applications so if you think you might ever want to more than just a pop-up or pop-down lift it might be worth your time to contact them.

The owner is easily accessible and they are up for the challenge of one-off design if you have a need for something custom.  I had a simple problem with one lift once installed (limit switch), and they had a tech drive up the next day to sort it out so I didn't have to mess with it myself.
