It's an OEM model that appears, from this site, to have been supplied with the LG LGXBR342 and Zenith XBR411.

I've found manuals for both of these models, here and here, but the former is incomplete and the latter appears to be a different model of remote. Perhaps, however, you can muddle through with a combination of both.
