I'm not sure who configures the firmware for cable boxes. If I understand correctly, the box is designed and manufactured to be very general for IR functions, so an individual cable company can make the box respond to different IR functions. But I don't know how much of that is set up for the cable company to configure themselves vs. needing support (new version of customized firmware) from the cable box manufacturer.
