I recently setup a Toshiba TV that sounds similar to yours. Initially I was always one input short of the desired video destination. I ended up telling it that it used the "input next" key to open a menu and then it used the "input next" key again to cycle through the inputs. This worked in my case.

I was initially one input short because if you manually push the button on the TV itself it would accept the first button push as an increment of the video inputs. Through the remote the first "input next" command only opened the menu and did not increment the video input.
