We hit "more activities" and pick "watch replay TV" and
then screen shows: "Watch Replay TV" at top and on left
side "Aspect" and "PgDown" and on the right side of the
display "PgUp" and "Live TV." What are we missing here?

Well, to begin, a Dish DVR is not a ReplayTV. When you set up the remote, did you add a Dish DVR or a ReplayTV? And you will need to add a few custom buttons to the Watch Dish activity.
