It's quite likely that the original Device profile for your TV had a custom input selection script/routine written by tech support. They quite often have to do this for TVs with oddball input switching methods that don't match any of the standard options. I had to get them to do this for my TV, and the solution they come up with becomes the standard profile for anyone who subsequently adds the same device to their setup. Tech support have access to much more advanced programming than we do as end users.
When you added the extra input it would have "overwritten" this custom script, leaving you with one of the standard 4 options (which obviously won't work).
You will need to phone tech support and get them to re-do the original script, or come up with a new routine for your particular set up. You will need level 2 or 3 tech support (in Canada) to do this.
