Thank goodness I found this post. Thank you edmund, worked perfectly.
I entered code 31254, but it didn't work until I did the 2nd set of steps:
1. av1 or av2
2. press and hold MUTE & SELECT for two blinks
3. enter 9 9 3
4. press SELECT
Just to help fellow Googlers, my Direct TV Receiver is model HR21-700.
