Long ago, I came in to fix a commercial job which went south.
The IT guy punched down all of the Cat5e jacks at the patch panel using T568B.
The electrician installed all of the wall plates throughout the building using T568A.
This resulted in nothing but a building filled with cross-over cables.
Ran into that in a house- we were told to use 568B for everything, but residential network modules for structured wiring had been spec'd and punched down 568A. The guy who was re-assigning the pairs at the module gave me a hard time about it, even though he was there when we were told which standard to use and he was the one who punched them down.
