One thing I learned from BellSouth years ago was: You never DRILL into a space when you don't know what is in the space.
I know someone (electrician) that learned a very expensive lesson by drilling through a floor to install a floor outlet, and then watching in amazement as a stream of water came shooting out of the floor.
He was just predrilling a hole for lagbolts to hold a tv bracket to the wall. In most cases, I trace the bracket position on the wall and then cut my single gang holes in first which gives me a peak inside. But I can't really blame him for drilling a small hole as he did. There should have been nail plates in place.
