FWIW, I have seen this happen once on a new system.
Card was bad, replacement card worked fine.

Also you can try pulling the card, blowing the slot out with canned air and reinserting it. Have had a few DM cards get flakey on me in somewhat dusty environments, blowing the dust out got those to behave again.
I am a trained professional..... Do not attempt this stunt at home.
