Troubleshooting something like this involves going down every possible blind alley to see if some pattern reveals itself.

It seems you mean that the Integra is already on. Or is the Integra switched on by CEC when you turn on the TV? Check every scenario below with the Integra on first; with the TV on first; with CEC on; and with CEC off.

When you turn on the TV and the Integra is set to DVD (not BD), do you get snow? Does changing the Integra to (unnamed input in above scenario) make it "work fine"? Your initial description contains switching away from (unnamed) and switching back to it. It would be nice to see if it's only that input, and if it might be switching away from something that makes it work right.

When you turn on the TV and the Integra is set to some audio-only external input, do you get snow? Does it then work okay when you switch to (unnamed input)?

Ditto with the internal FM tuner? This sounds ridiculous but it's gathering information.

What happens if you turn on the TV and then turn on the Integra? Now go through those combinations above.

If that cures things, you can think of turning on the Integra as changing the inputs. After all, when you turn it on it changes from no input to the (unnamed) input on the Integra.

I may have duplicated some things above. Check it all.
All firmware on all devices is up to date.

I'm guessing this is an issue with the Integra. It's been happening according to the client from day 1.

Under exactly what wiring setup, CEC setting, and turn-on sequence?
Does anyone have any solutions?

Try those things I listed to see if any of them don't create this behavior.
