When I was a busser at a restaurant many moons ago. I had witnessed a few disturbing things.

One was when the toast landed on the ground, butter side was between the slices. And the waitress took it out to the customer anyways. I was too young and naïve to say anything at the time. According to her, which was true, it was on the floor for less than a second. That was her reasoning to let it slide.

At the dishwasher station we had the garbage can for the left over scrap and such. When it gets about half full, one of the dishwashers would empty it. What I saw him do completely freaked me out. I watched him wipe the edge with his fingers and stuff his face with it. I was so shocked that I couldn't say anything.
