Always include your area code. Assuming the recipient will know your area code is a bad idea in this day and age, and many carriers require it for local calls.
I prefer the 702-300-0101 format, dropping the more often than not superfluous 1 at the beginning. I'm not sure when or why people started using dots (i.e 702.300.0101); likely to be cute or make themselves seem super technical geniuses; we are all impressed.
