Need more info.

What type of ISP are you working with? It could be that even though you have the port open on the Dlink that the ISP's device has it closed. I see this happen alot with DSL modems. You have to have most DSL modem's setup in bridgemode. If it is serving up DHCP addresses it won't work unless you go open up the ports in the DSL Modem as well.
